I too have cleared this boss multiple times and no Skull drop.
Bumping this thread because this is STILL an issue and is pretty game breaking for anyone who needs spirit for their builds. Crazy that this hasn't been addressed yet when it's such an integral part of the game (actually being able to use certain spells/skills that rely on a base spirit threshold.)

I didn't receive the gembloom skull on Act 1 Cruel, and my fiance has killed the king in the mists on Act 1 normal and Cruel, neither time did he get the drop.

Please FIX THIS.
I've just spent a load of Ex orbs for a build and then noticed i didn't get any spirit from this boss! now i can't use my build. I've redone the boss 4 times and still no drop
same thing happened to me. im already at act 3 without spirit ssst
Still happening. Killed him in Cruel, no drop. As hard as it is to get Spirit this is a pretty shitty bug
I've been looking into this and I'm trying to determine how much is confusion or a potential bug based on what people have said. Its a little difficult to be certain so I just wanted to give a brief overview of the Spirit quest items.

We intentionally expect Spirit quest items dropped in the campaign to work as follows:
"
- Mist King Act 1 Normal -> 30 Spirit item
- Ignagduk Act 3 Normal -> 30 Spirit item
- Mist King Act 1 Cruel -> No Spirit item
- Ignagduk Act 3 Cruel -> 40 Spirit item

The base total of Spirit you should have for clearing the above bosses should be 100. Cruel Mist King does not drop a spirit item which is a result of using a Difficulty that repeats the first three acts, and I believe it would be both easy and understandable to assume that he would drop another Spirit item. This specific point of confusion will hopefully be remedied once the rest of the acts are added.

Aside from the potential confusion caused by the above, it does sound like quite a few people aren't getting Spirit quest item drops from some of the bosses or spirit from consuming the items. If your experience and base Spirit totals don't match up with the above list it would be immensely helpful if you could post a screenshot of your current Maximum Spirit without items and which bosses you have cleared.

If there is an issue for people getting the expected Spirit beyond potential confusion we certainly want to resolve it.
Happened to me too I just defeated executioner and beat king of mist a while ago but never received any item that adds spirit or consumable. My current spirit is still at zero.

Worth splitting into 20 and 20 across Cruel King otM and Cruel Ignagduk?

I would suggest just giving the player the bonus spirit on kill and making the boss just drop a guaranteed spirit gem. Dropping an item to pick up to use is very error prone. Would be better if it was an NPC quest reward at least like most things in 1 cause it's less susceptible to player error or server crashes etc.
