I wanted to share my experience with the same issue regarding the Gembloom Skull and Spirit bonus.

My friend and I were playing as a duo in Act 1, and here's what happened:

I had already completed the Rituals quest previously while playing solo and had received the Gembloom Skull, which grants +30 Spirit.
When we attempted the quest together, only my friend needed the Spirit bonus. We completed the Rituals, defeated the King in the Mists in Freythorn, but the Gembloom Skull did not drop for my friend.
My friend tried redoing the quest solo multiple times after this, but the item still didn’t appear.
Unfortunately, I cannot recall who was the group leader at the time, but I suspect it was me. This might be relevant to how the game determines who is eligible for the quest rewards.
It seems like playing in a group where one player has already completed the quest might be causing the issue. This bug is particularly frustrating because the Spirit bonus is essential, and there seems to be no way to fix it through replays.

I hope this feedback helps with debugging the problem. If anyone has found a workaround or solution, I’d love to hear about it!

I would agree this could very well be the issue that if grouped with others could mess things up. But since there is no Solution for this issue and you can't start a new account and it is very essential this should be worked on and fixed as soon as possible. I can't imagine starting over and that doesn't even work then what is the point?
